To print out this sheet, place your cursor on the picture, and hold it there until the
print button pops up, then press it. If that does not work, save the picture, and print it out in a different program. Remember,
if you change your printer setting to landscape before you print it, you may be able to fit 2 fill-out sheets on the page
if you take the page with the already printed form, and flip it upside down to print again.
